Who this master class is for

The «Botulinum Therapy for «Difficult Faces». Complications of Botulinum Therapy» master class is for doctors who already work with botulinum toxin and meet patients for whom standard schemes do not fit: with asymmetry, a narrow forehead, a wide nasal bridge, particular aponeurosis anatomy. The second half of the title matters just as much: a large share of the time is devoted to side effects and complications — how to prevent them and what to do if they occur.

It is one of the three topics of the «Practical Dermatocosmetology» cycle, together with prevention of complications in contour plastics and the algorithm of comprehensive facial rejuvenation.

Asymmetry and «difficult faces»: what is analysed

Expression lines form where facial expressions are most active: over time the muscle tension becomes permanent. To relax such chronic spasms, cosmetologists use botulinum toxin injections. In microdoses, botulinum toxin type A is also widely used in medicine — in ophthalmology for blepharospasm, to relieve migraine, to reduce sweat gland activity. It is a potent substance, so the choice of dose, injection points and methods is treated at the master class as a topic of its own, not as a line in a scheme.

The theoretical part starts with diagnosis:

• the concept of asymmetry — physiological and functional;
• the concept of myofascial syndrome and its link with asymmetries;
• examining the patient: neurological status, checking cranial nerve activity;
• deviation of the eyeball in congenital weakness of the adducting eye muscle.

Then comes choice: the right choice of patient, of product for the specific patient, of dose, injection points and methods. Injection options for different anatomy are analysed separately — aponeurosis structure, a narrow forehead, asymmetry, a wide nasal bridge and so on.

Complications of botulinum therapy and how to deal with them

The complications block is built on real clinical cases. The most common side effects of botulinum toxins are analysed:

• ptosis;
• an overhanging nasal bridge;
• the cheek «sail».
